Regulation on renaming user of gas

1.Regulation and operational process for changing residential user of gas into another residential user

1.1.Acceptation Scope
This regulation is applied to a (residential) users of gas who lives in the region administrated by the Shanghai Dazhong Gas Company Limited and wants to change his (or her) name in gas account book into another residential user. The existing user and the new user shall go to the regional office with jurisdiction together to file an application.

1.2.Making application

1.2.1.The existing user shall bring his (or her) ID card, permanent residence booklet or effective certificate of property (which proves this is not a temporarily rented residence) and latest gas bills that have been paid. The new user shall bring his (or her) ID card, permanent residence booklet or effective certificate of property (which proves this is not a temporarily rented residence) for making an application.

1.2.2.If someone is trusted by the users for the application of rename, the consignee shall present his (or her )ID card and the consigner’s ID card as well as a writing consignment certificate. The consignment certificate shall be received by staff member for examination and record.

1.2.3.The users shall correctly complete the “Application form for gas work” and “Agreement on renaming user of gas” and sign on them for confirmation. If the application is made by a consignee, the consignee shall fill his (or her) ID card number and shall sign too.

1.2.4.At the moment of renaming user, the existing user must have paid all issued gas bills, and the present gas meter reading which has been confirmed by both existing and new user, or the last gas meter reading made by the Company, shall be taken as a baseline for further meter reading after renaming and shall be filled in the “Agreement”. (Note: A bill shall be issued for paying off the present gas meter reading when the rename application is made).

1.3.Provisions of application

1.3.1.The gas facilities of existing user can only be transferred to a new user who lives in the same residence (but who is not one of the members listed in the same residence booklet or effective property certificate), or only be transferred to the head of household or other members who are listed in the same residence booklet or effective property certificate. (In the later case, there shall be a writing consenting certificate made by all members listed in the same residence booklet or effective property certificate, and this certificate shall be kept by staff member for record).

1.3.2.During renaming the user of gas, the new user shall pay the fee of gas facilities or “Contact coupon for former user” and the fee of handling business. However, the new user shall only pay the fee of handling business and not pay the fee of gas facilities or “Contact coupon for former user” in one of the following situations.

1.3.2.1.Rename is made for a person who is listed in the same residence booklet or effective property certificate. (The existing user cannot ask for a “Contact coupon for former user”.)

1.3.2.2.The registered permanent residence of new user was transferred into this address in a date earlier than 1st day of July, 1990.

1.3.2.3.As the registered permanent residence of existing user have been transferred out of this address after 1st day of January, 1989 since the renting relationship of residence was changed, during the transfer of registered residence the existing user expressed in a written form that he (or she) did not want to have a “Contact coupon for former user”.

1.3.2.4.The ownership of gas facilities is defined in the legal documents for transferring (by transaction or donation), auctioned or judicative property (such as property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper); or the ownership of gas facilities is not defined in the legal documents for transferring (by transaction or donation), auctioned or judicative property ( such as property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper) but the exisitng user did not want to have a “Contact coupon for former user”.

1.3.2.5.Two divorced spouses want to rename the user of gas and the ownership of gas facilities was defined in divorce judgment or agreement; or the ownership of gas facilities was not defined in divorce judgment or agreement, but the existing user did not want to have a “Contact coupon for former user”.

1.3.2.6.As for a transferring (by transaction or donation), auctioned or judicative property, it is impossible to contact the existing user, but the new user is willing to pay all gas bills unpaid by the existing user. (The new user has to conclude an agreement with the accounting center of the Company for handling this issue. If the existing user appears later and asks for a “Contact coupon for former user”, then the existing user shall refund the new user for all payments the new user has made in respect to gas bills, the fee for gas facilities and “Contact coupon for former user”).

1.3.2.7.Other particular cases shall be approved by competent superior authorities.

1.3.2.8.The existing user whose registered permanent residence was transferred from this address at a date earlier than 1st day of January, 1989 since the renting relationship of residence was changed, shall not ask for a “Contact coupon for former user”.

1.3.2.9.After the renting relationship of residence is changed, the user who is not listed in the same registered residence booklet or effective property certificate ask for renaming the user of gas, but it is impossible to contact the existing user due to some definite reasons (such as death, staying abroad, leaving from Shanghai for many years), so that it is difficult to make sure that the existing user would like to have a “Contact coupon for former user” or not. Therefore the new user may apply for renaming user by presenting his (or her) ID card and relevant certificates in accordance with the Article 3.2 of the regulation. (The photocopies of relevant certificates shall be kept by staff member for record.)

1.3.3.If a residential user who uses the name of unit as the user name administrated by a regional office specified for residential user (but the price of his gas consumption is residential price) wants to rename, he (or she) shall present the consent certification of unit and his (or her) ID card for making application; if the unit does not exist now, a consent certificate issued by property administrative authorities may be presented too. Then the application of renaming user shall be handled in accordance with the Article 3.2 of the regulation. (The photocopies of relevant certificates shall be kept by staff member for record.)

1.3.4.If the ownership of gas facilities is not defined in the legal documents for transferring (by transaction or donation), auctioned or judicative property (such as property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper) both parties shall bring their ID cards to go to regional office for making application in accordance with the Article 3.2 of the regulation. If the ownership of gas facilities is defined in the legal documents for transferring (by transaction or donation), auctioned or judicative property (such as property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper) the new user shall bring property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper and his (or her) ID card to a regional office for making application in accordance with the Article 3.2 of the regulation.

1.3.5.If two divorced spouses want to rename the user of gas and the ownership of gas facilities is not defined in divorce judgment or agreement, both parties shall bring their ID cards to go to regional office for making application in accordance with the Article 3.2 of the regulation. If the ownership of gas facilities is defined in divorce judgment or agreement, the new user shall bring the photocopies of relevant certificates and his (or her) ID card to a regional office for making application in accordance with the Article 3.2 of the regulation. (The photocopies of relevant certificates shall be kept by staff member for record.)

1.3.6.The fee of handling business and the fee of gas facilities shall be collected in accordance with prevail “Summary of Prices of Gas and Service in Shanghai” and “Reply on Consenting to Collect the Fee of Gas Facilities”.

1.3.7.If the name of “Contact coupon for former user” handed in by the new user is different with that of new user, the new user shall be asked for paying the fee of gas facilities, or providing relevant certificates and renaming the “Contact coupon” in accordance with the regulation on management of “Contact coupon for former user”.

1.4.Operational process

1.4.1.Making application: User shall bring relevant certificates to regional office with jurisdiction for application. Staff member shall ask the user to complete an “Application form of gas work” and give the user a copy of “Guideline for safety of gas user”.

1.4.2.Application examination: The staff member of regional office shall examine the information of user on computer to make sure that the user has not any unpaid gas bill and check up all relevant certificates provided by the user. .

1.4.3.Inform after examination: If the certificates do not meet relevant requirements, the user shall be informed the detailed requirements for certificates. If the certificates meet relevant requirements, the user shall be informed that the application has been accepted. Moreover, the user shall be asked to complete an “Agreement for renaming user of gas”. (The staff member shall check up the contents filled; if rename is made in the same household and collection via bank is reserved, the seal “I want to reserve collection via bank” shall be stamped on the agreement and the user shall sign on it too.) Relevant certificates shall be kept for record in accordance with relevant regulations.

1.4.4.Collect the fee of gas facilities (or “Contact coupon for former user”) in accordance with relevant regulations.

1.4.4.1.The staff member shall issue an invoice for the fee of gas facilities with the name and address provided by the new user; and the user shall pay at the counter of cashier.

1.4.4.2.The staff member shall check the household name on “Contact coupon” and the name of applicant; if both names are the same, accept the “Contact coupon”. If two names are different, the user shall be required to pay the fee of gas facilities, or rename the “Contact coupon for former user” by providing relevant certificates in accordance with the provisions of management regulation of “Contact coupon for former user”.

1.4.4.3.Based on information shown on computer, the staff member shall check whether the user has paid all issued bills. If the user has any arrear, the staff member shall issue a bill and ask the user to pay the amount of arrear at cashier counter. If the existing meter reading has been confirmed by both new and existing user in this rename process, the staff member shall issue a bill and ask the existing user to pay this bill at the counter of cashier. Meanwhile the staff member shall check up the baseline of meter reading on the rename agreement.

1.4.4.4.The cashier shall collect money in accordance with the invoices and bill issued by the staff member, and also shall collect the fee of handling business from the user. Stamp “Paid” seals on all invoices and bills before returning them to the user.

1.4.4.5.After the user has paid, the staff member shall check up all “Paid” seals on invoices and bills.

1.4.5.The staff member shall enter the information of rename into computer (If collection via bank is reserved, the information shall be transferred to accounting for treatment).

1.4.6.The staff member shall find the corresponding meter reading card in three workdays and fill the rename date and the name of staff member, replace the name of existing user with the name of new user. If the existing meter reading has been confirmed by two parties in this rename process, make corresponding records on working card.

1.4.7.Keep on files: The staff member shall hand over all files ( such as “Application form for gas work” and “Agreement on renaming user of gas”) provided by user to the head of working team in three workdays. Prior to the 30th day of every month, the head of working team shall keep on these files received from the 26th day of the last month to the 25th day of this month in archive. (This work may be delayed if there is any public holiday.)

1.4.8.Before the 30th day of every month, the head of working team shall report the number of rename issues handled from the 26th day of last month to the 25th day of this month to the safety and service division of business office (It may be delayed correspondingly if there is any public holiday). Before the 5th day of the next month, the safety and service division of business office shall summarize all statements handed in. (It may be delayed correspondingly if there is any public holiday)

2.Regulation and operational process for renaming residential user with the name of unit user

2.1.Acceptation Scope: A residential user of gas who live in the region administrated by the Company wants to change the name of user in gas account book into the name of unit. Then both parties shall go to a regional office with jurisdiction together to file a rename application.

2.2.Making application

2.2.1.The residential user shall bring his (her) ID card or permanent registered residence booklet and latest paid gas bills. The unit user shall bring business license, effective property certificate and the ID card of the representative of unit.

2.2.2.If someone is trusted by the residential users for application of rename, the consignee shall present his (or her ) ID card and the consigner’s ID card as well as a writing consignment certificate. The consignment certificate shall be kept by staff member for examination and record.

2.2.3.The users shall correctly complete the “Application form for gas work” and “Agreement on renaming user of gas”, and sign on them for confirmation. If the application is made by a consignee, the consignee shall fill his (or her) ID card number and shall sign too. Moreover, the representative of unit shall also fill his (or her) ID card number and sign on the Agreement.

2.2.4.At the moment of renaming user, the residential user must have paid all gas issued bills. Moreover, the present gas meter reading which has been confirmed by both existing and new user, or the last gas meter reading made by the Company, shall be taken as a baseline for further meter reading after renaming and shall be filled in the “Agreement”.

2.3.Provisions for application

2.3.1.The gas facilities of residential user shall only be transferred to a unit user at the same address (residence).

2.3.2.If it is clear that the residential user is going to rename with a unit user, it will be prohibited to accept an application made by residential user for installing a meter with larger flow rate.

2.3.3.The unit user need not to pay the fee of gas facilities in rename process, but the unit has to pay the fee of handling business and cost of facility card to a regional office.

2.3.4.In the rename process, the residential user shall pay the handling fee for rename operation. The regional shall not issue any “Contact coupon for former user” (Any compensation issue between both parties shall be settled by themselves through negotiation.)

2.3.5.If the ownership of gas facilities is not defined in the legal documents for transferring (by transaction or donation), auctioned or judicative property (such as property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper), both parties shall go to regional office for making application in accordance with above provisions for application and regulation. If the ownership of gas facilities is defined in the legal documents for transferring (by transaction or donation), auctioned or judicative property (such as property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper), only the unit user shall bring property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper and the business license of the unit as well as the ID card of representative of unit to regional office for applying for rename. (The photocopies of property purchase contract, donation certificate with notarization, auction certificate or legal judgment paper shall be kept by staff member for record). Meanwhile the unit user not only shall pay the fee of handling business and cost of facility card, but also shall pay the handling fee for rename operation to regional office.

2.3.6.If it is definitely impossible to contact the residential user due to some reasons (such as death, staying abroad, leaving from Shanghai for many years),(resulting in that it is difficult to confirm whether the right and interest of the residential user have been assured or not.), the unit user may apply for rename solely by presenting relevant certificates, the certificate of property right (i.e. non-rented residence), writing commitment statement for assuring the right and interest of residential user, business license and the ID card of representative of unit. (The photocopies of relevant certificates and writing commitment statement shall be kept by staff member for record). Meanwhile the unit user shall not only pay the fee of handling business and cost of facility card, but also pay the handling fee for rename operation for residential user in a regional office.

2.3.7.The fee of handling business shall be collected in accordance with the prevail “Summary of Prices of Gas and Service in Shanghai”。

2.4.Operational process

2.4.1.Making application: User shall bring relevant certificates to a regional office with jurisdiction for residential users to file an application. Staff member shall ask the user to complete an “Application form for gas work”.

2.4.2.Examining application: staff member shall check the information of this user on computer to make sure that the user has not any unpaid gas bill. Meanwhile the staff member shall examine the certificates provided by the user.

2.4.3.Inform the user after examination: If the application or certificate does not meet relevant requirements, the staff member of regional office shall inform the user what are the necessary requirements a user has to meet. If relevant requirements are met, the user shall be informed that his (or her) application has been accepted. The user shall be asked to complete an “Agreement on renaming residential user (for an unit)” (of which the filled contents shall be checked up by the staff member) and relevant certificates shall be kept as required.

2.4.4.Collect the fee of handling business and all bills in accordance with relevant regulations.

2.4.4.1.Based on information shown on computer, staff member shall check whether the user has paid all issued bills. If the user has any arrear, the staff member shall issue a bill and ask the user to pay the amount of arrear at cashier counter. If the existing meter reading has been confirmed by new and existing user in this rename process, the staff member shall issue a bill and ask the existing user to pay this bill at the counter of cashier. Meanwhile the staff member shall check up the baseline of meter reading in the rename agreement.

2.4.4.2.The cashier of regional office shall collect money in accordance with the invoices and bill issued by the staff member, and also shall collect the fee of handling business from the user. Stamp “Paid” seals on all invoices and bills before returning them to user.

2.4.4.3.After the user has paid, the staff member shall check up all “Paid” seals on invoices and bills.

2.4.5.The staff member shall enter the information of user rename into computer right away.

2.4.6.The staff member shall find out the corresponding meter reading card in three workdays, fill the rename date and the name of staff member and replace the name of original user with the name of new user. If the existing meter reading has been confirmed by two parties in this rename process, make corresponding records on working card.

2.4.7.Every Thursday the head of working team shall fill in “Summary list of renaming residential user for unit” and “Summary list of renaming residential user (for unit) and gas facilities” by referring to the cards of meter reading collected from the last Thursday to the Wednesday of this week. These lists shall be delivered to the safety and service division of business office. (This work may be delayed correspondingly if there is any public holiday).

2.4.8.Every Friday the safety and service division of business office shall summarize all lists and statements submitted by regional offices and then fill in the “Summary list of renaming residential user for unit user” and “Summary list of renaming residential user ( for unit) and gas facilities”, which shall be handed to the marketing department of the Company.

2.4.9.After receiving the PC code of new user (unit) fed back by the marketing department of the Company, the safety and service division of business office shall transfer the PC code and the card of facilities to the head of working team in the regional office that handled this application in 2 workdays.

2.4.10.After receiving the PC code of new user (unit) and the card of facilities that are transferred by the safety and service division of business office and fed back by the marketing department of the Company, the head of working team in the regional office shall enter the relevant information of user using a meter for less 6 M3/h (including 6 M3/h) into PC and shall make out a new meter reading card, and then hand the new and old meter reading card together to the head of meter reading team. This work shall be finished before the date when the meter reading team rewrites the cards of meter reading by the end of every month. As for the user using a meter for over 6 M3/h, the head of working team shall work out a new meter reading card by referring to received information, fill the readings of last three months and the parameters of the meter into the new meter reading card, and work out a summery of transferred meter reading cards. The head shall hand the meter reading cards and summery to the safety and service division of business office and return the former meter reading cards to the head of meter reading team in the regional office. (Proper sign for confirmation is required)

2.4.11.After receiving the meter reading cards and summery transferred by regional office, the safety and service division of business office shall deliver these files to the head of meter reading team in the same workday. (Proper sign for confirmation is required)

2.4.12.The head of meter reading team shall insert the new meter reading card into the meter reading book for unit user in the date specified for rectifying meter reading cards by the end of every month. He shall make records properly on the meter-reading book and keep the former meter reading cards in files orderly.

2.4.13.By the end of every month, the head of working team shall collect all relevant information that has to be kept on, “Application form for gas work”, “Agreement on renaming user of gas” and the business information fed back by the marketing department of the Company, and keep them in archive orderly.

2.4.14.Prior to the 30th day of every month, the head of working team shall report the number of renamed users form the 26th day of previous month to the 25th day of this month to the safety and service division of business office. (This work may be delayed if there is any public holiday). The safety and service division of business office shall summarize all reports submitted by regional offices before the 5th day of the next month. (This work may be delayed if there is any public holiday).
